This survey is short (30 questions), easy to use and will provide our consultation team with valuable perspectives about our church. The following instructions will assist the you in completing this survey tool as a part of the Step 2 process of the Fruitful Congregation Journey:

2.) Goal: We are hoping to have about 70% of our average worship attendance (not membership) complete the survey.
3.) Deadline: October 1, 2015
All data submitted through the online survey tool is completely confidential.
If you do not have a computer, a paper copy is available in the church office. This copy must be returned to the office so it can be entered into the system.
Each question must be answered. If unsure of an answer you may choose ”neutral” for fill in the blank questions or “none” for text answer questions.
The results of the completed surveys for each church will generate a report that will be used by the Weekend Consultation team and may be referred to in the Weekend Assessment Report for each congregation.
